Registration Information
To register by fax or mail, print off a copy of the paper registration form and return a completed copy to our office. Faxed registrations must include credit card number and signature; keep your original if you register by fax. Payment must accompany registration. Payments are accepted via VISA, Amex, Discover, MasterCard, check/purchase order or University of Michigan shortcode. Make checks/money orders payable to University of Michigan and mail them to Marketing and Conference Services, 627 Oxford Road, Vandenberg Hall, Ann Arbor, MI 48104-2634.

Meals:
The registration fee includes meals. Please make sure to indicate in the appropriate areas what meals you plan on attending.

Confirmation

Registrants will receive a confirmation by email, fax, or mail within 7-10 days of registering. (Please note: Those registering online will receive an automatic acknowledgment of their registration, but this is not the official confirmation.) If you do not receive a confirmation within 10 days of registering, please contact 734.764.5297, or email conferences@umich.edu.

Cancellation Policy:
Written cancellation notification via mail or fax must be received no later than May 15, 2008 to qualify for a refund, less $35.00 administrative fee. Those who register, but fail to cancel by the deadline and do not attend the conference, will not be eligible for a refund. Refunds will take approximately six to eight weeks. Substitutes are encouraged - please notify us as soon as possible.
